The Occupational Safety and Health Administration has cited a Weston, Wisconsin-based manufacturer for a serious and two repeated violations of machine safety failure following a serious workplace injury.
An employee working at Crystal Finishing Systems Inc.’s Aluminum Extrusion Plant in Weston was taken to hospital with serious injuries after being hit by a puller as he tried to loosen a piece of aluminum.
OSHA investigated the incident on December 21, 2021 and determined that 6-foot chain link guards were not attached around the 8-inch extrusion press line to prevent workers from coming into contact with the puller. In addition, the press line was not locked to prevent accidental machine movement during service and maintenance, and the employer failed to train workers in machine safety procedures.
OSHA cited Crystal Finishing for a serious and two repeated violations and proposed penalties of $ 159,522. The company was prosecuted for similar machine safety violations eight times between 2012 and 2019.
