Inspectors claimed that the manufacturer failed to use machine protection, did not carry out regular inspections and failed to ensure that workers wore protective eye or face equipment and hearing protection.
The company has 15 working days to dispute the results.
D&D Manufacturing did not respond to a request for comment.
