PUBLIC SECTOR REMINDERS

1. Recording Criteria – Follows the Recording Criteria as Description CFR 1904
2. Recording Form – Document Usage Form and W10 can be found in the above package link SBD10710 (contains the same as available on OSHA Form 300 and 300A)
3. Requirement for electronic submission – Expires March 1
   1. eSLA Customer portal login
   2. If not provided by 3/1, orders from DSPS can be issued and can perform security checks
4. Posts – after completing the electronic submission an e-mail confirmation with print version for posting
