Jeffrey Brian Cohen, 39, of Reisterstown, Md., After four days of trial, admitted guilty on 5 June 2015 of fraud, aggravated identity theft, making false statements to an insurance authority and obstruction of justice.
According to the information in his founding agreement, Cohen served as president and chairman of the board. from a Delaware company called Indemnity Insurance Corporation RRG. Cohen previously controlled a District of Columbia company called the Indemnity Insurance Corporation of DC, Risk Retention Group (Indemnity-DC), which was a forerunner of Indemnity.
Iowa Agent receives 41
months in prison for fraud insurance companies
Melissa Ilene Williams, from Dike, Iowa, was ordered to pay compensation for creating and filing life insurance policies to two separate insurance companies between July 2011 and January 2012. Williams admitted guilty of a number of postal frauds on March 3, 2014
Williams, a former Des Moines, Iowa, insurance agent was sentenced to 41 months in prison for insurance fraud, the Iowa Insurance Division announced.
The Iowa Insurance Division revoked Williams' insurance producer license in November 2013.  Williams was ordered in federal court to pay a full refund of $ 139,137.13 to Funeral Directors Life Insurance Co., a Texas-based insurance company, and $ 36,010. 19 to Nat ional Guardian Life Insurance Co. in Madison, Wis.
When she is released, Williams will also be monitored by the authorities for another three years.
The insurance department said the investigation showed that in 2011 and 2012 Williams forged signatures of people she knew on fake life insurance policies and collected $ 170,292 in fraudulent commissions from Funeral Directors Life Insurance Co. and National Guardian Life Insurance Co.
Williams was convicted on June 19, 2014 in the U.S. District Court for the Northern District of Iowa in a lawsuit filed by the United States Attorney & # 39 ;s Office for the Northern District of Iowa. The indictment was the result of a collaboration between the Federal Bureau of Investigation, the United States Postal Inspection Service and the Iowa Insurance Fraud Bureau.
